SETTING UP A MEMORIAL FUND

CHARITYSMITH is pleased to help you set up a memorial fund to honor your loved one.

The following are items requested to create the Memorial Fund through CHARITYSMITH. They are generally listed in the order of importance for the successful creation of the memorial fund website.

To complete the process, please fill out and submit the Build A Fund Worksheet:

  1. Name of the Memorial Fund as it should appear on documentation.

    (i.e., Douglas F. Smith Memorial Fund)
  2. Description of the Memorial Fund. (We suggest a brief two part description mentioning the person memorialized and the intended beneficiaries; however this is merely a suggestion.)

    (i.e.,  The Douglas F. Smith Memorial Fund, established in 1994, is a tribute to the memory of Doug Smith.   Distributions are awarded annually to a student, artist, or athlete in support of their efforts to pursue a given aspiration. As a general guideline, recipients have demonstrated an understanding of excellence as a function of persistence and hard work which exemplifies the concept of “success” as a prize earned in monotonous and unrecognized moments, where often alone and without witness, the victor earns his right to victory.)
  3. Give three suggestions for the preferred URL, if you have opted to have CHARITYSMITH create a webpage for the Memorial Fund. (The shorter the URL, the better, however this is merely a suggestion.)

  4. Fund Adminstrator(s):

  5. Acknowledge Compliance
    The fund must work in conjunction with CHARITYSMITH Nonprofit Foundation's stated purpose as indicated by commission from the State of California and the Internal Revenue Service Code under section 501(c)3.  CHARITYSMITH's commission is "the creation, administration, and distribution of memorial funds." All financial awards from a CHARITYSMITH governed memorial fund are reviewed by the Board of Trustees to ensure compliance with this stated purpose.
